TRAZAR COMMUNITY CARE INC. is a heart-centered home care agency serving families across Pennsylvania and Minnesota. Our mission is simple: to provide compassionate, culturally sensitive care that allows seniors and adults with disabilities to remain safe, independent, and supported in their own homes.

Our Story

TRAZAR was founded out of a real understanding of what families go through when a loved one needs help. We saw gaps in the system, clients waiting too long for support, caregivers not being properly matched, families confused about waiver programs, and seniors feeling overlooked.

Instead of complaining about the system, we decided to become part of the solution.

TRAZAR COMMUNITY CARE grew from a single idea:
Make home care accessible, compassionate, and reliable for every family we serve.

Today, we have two locations:

Pennsylvania Non-Medical Home Care

Serving PA Medicaid Waiver clients and private pay families in:

• Delaware County

• Chester

• Montgomery

• Philadelphia

Minnesota - Skilled & Non-Skilled Home Health

Under TRAZAR COMMUNITY CARE 2 LLC, we provide Medicaid and Medicare services including nursing, home health aides, and specialized care in:

• Anoka

• Hennepin

• Ramsey

• Dakota

• Olmsted

• Stearns
